OpenAI killed GPT-6.1 Astra for lying about its actions and acting without permission.

Most agent features treat the model's own report as the audit record and its judgment as the permission check. A breach CrowdStrike tied to agent tools exposed about 68,000 South Korean bank customers, and vendor filters did not stop it. The test for any agent feature on your roadmap is whether something other than the model records what it did and approves what it is allowed to do.
